
A written order from your physician is required for all imaging exams. The order may be written on our referral form or that of any other radiology practice, or the physician’s own prescription form. If you do not bring the order with you and we cannot obtain one from your physician’s office, we will need to reschedule your exam. In addition, please bring your most current insurance identification card(s) for both primary and secondary insurances and photo identification. These are required to submit the claim to your insurance company.
If you find that you cannot attend your appointment, please call us as early as possible in order to offer the time for another patient. If you are late for your appointment, we will make every effort to work you in. However, it may be necessary for you to wait or reschedule your appointment.
All patients are asked to arrive 30 minutes early in order to complete their registration and history forms. To save yourself wait time, please take the time now to download the forms from this web site and complete them in advance of your arrival.
Please do not bring children who are not accompanied by another adult to your appointment. Siblings are welcome to attend the obstetrical ultrasound exam if accompanied by an adult and safely secured in a stroller or baby seat. For the comfort of all patients and due to safety regulations, children should not be allowed to wander the office or disturb other patients.
Please help us maintain a peaceful and clean office environment by observing our signage. Cell phone use is prohibited. Please do not bring food or beverages into the office.
